What it means

The signal from the upstream oxygen sensor on bank 1 stays flat near its bias voltage of roughly 0.45 V and never crosses the rich and lean thresholds. The ECM therefore has no usable feedback and keeps fuel control in open loop, running the engine from stored maps only. Consequences are a lit MIL, elevated fuel consumption, unstable idle and emissions well above the legal limit.

Likely causes (most common first)

  1. Failed or completely worn out oxygen sensor
  2. Blown fuse or open circuit in the sensor heater supply
  3. Broken signal or ground wire between sensor and ECM
  4. Corroded connector or melted harness against the exhaust pipe
  5. Sensor never reaching operating temperature because of very short trips
  6. Faulty engine control module input stage

The engine still runs, but without lambda feedback consumption and emissions rise sharply, so repair soon.
